Our Community Health Center is a one-stop clinic for primary care, behavioral health, and community services — all together in one place for families across Alameda County, no matter their insurance status. A safe space for anyone who comes to us, the Community Health Center is our answer to addressing significant barriers to timely, high-quality, accessible, culturally responsive, and comprehensive healthcare. By bringing care together under one roof and offering services in the primary languages of our patients, we are working to make health care more accessible, connected, and responsive to the communities we serve.

When to seek emergency medical attention:
Look for emergency warning signs*. If you’re showing any of these signs, seek emergency medical care immediately:
- Trouble breathing
- Persistent pain or pressure in the chest
- New confusion
- Inability to wake or stay awake
- Bluish lips or face
*This list is not all possible symptoms. Please call your medical provider for any other symptoms that are severe or concerning to you.
Call 911 or head to your local emergency facility if you have any of the “emergency warning signs” listed above.
Some of these recommendations include:
- Stay home except to get medical care
- Take care of yourself – get enough rest and pay attention to your symptoms
- Stay in touch with your doctor and follow their instructions
- Separate yourself from other people
- Wear a mask unless you are alone
- Wash your hands
